Registration Form:

For the cost of $60.00 your child will receive a members kit (as shown in the picture), 20 program sessions and an allergy sensitive snack.

Children will receive an enriching, Christ centered program that allows them to develop in faith and friendship by learning Catholic values.
Your child will be apart of a program that is operating in over 150 parishes and schools in US and Canada with over 3500 members, 200 volunteers and 500 teen leaders.

The cost is $60.00 and you may pay for your child at Annunciation Parish. Please see Tess Perez at the Parish office. For $60.00 you will receive a members kit (as shown above which includes a T-shirt, bag and much more), 20 program sessions and an allergy sensitive snack at each session from September 2016 to June 2017
You must complete the registration form (online below) and parent permission form (which is attached and to be returned to the parish when payment is made).
To reserve your spot, you may fill in the registration form below (Please include the name of the full parent, the full name of the child/children registering (please include their current grade) and a phone number to contact you. You will receive a confirmation email (within 3 days). If your child has an allergy or medical concern we should be aware of, please let us know in the message box stating other.
